To maintain eligibility for this policy, several conditions apply.
- The necessary repair or replacement procedure must be performed at our office.
- The tooth structure itself must be sufficiently healthy and treatable to support continued restoration.
- Patients are required to attend at least two dental hygiene appointments per year following the placement of their crown or veneer; these cleanings do not necessarily need to be performed at our facility.
- It is important to contact us promptly regarding any dental problems related to the treatment or issues discovered during routine cleanings.
Please note, this policy does not cover damage resulting from recurrent dental decay experienced by the patient.
